Brand context & FDA status

Dulaglutide (brand name Trulicity, Eli Lilly) is a long-acting GLP-1 receptor agonist FDA-approved on September 18, 2014 for type 2 diabetes. In February 2020, the FDA expanded the label to include cardiovascular risk reduction in adults with type 2 diabetes with or without established cardiovascular disease — a notable distinction broadening the cardiovascular population eligible for treatment.

Mechanism of action

GLP-1 receptor agonist linked to an Fc fragment of human IgG4, conferring a long half-life (~5 days) and enabling weekly subcutaneous administration. Same downstream effects as other GLP-1s: glucose-dependent insulin secretion, glucagon suppression, slowed gastric emptying, central appetite suppression.

Clinical efficacy data

AWARD clinical program. A1C reductions of approximately 1.0–1.5% across doses. REWIND cardiovascular outcomes trial (Lancet 2019): 12% reduction in major adverse cardiovascular events in adults with type 2 diabetes and additional CV risk factors, including notably patients without established CVD — broadening the indication.

FDA-approved: glycemic control in adults with type 2 diabetes, and cardiovascular risk reduction in adults with type 2 diabetes with or without established CVD. Not approved for weight loss.

Dosing & titration

Weekly subcutaneous injection. Starting dose 0.75 mg weekly. May increase to 1.5 mg weekly for additional glycemic control. Additional doses of 3.0 mg and 4.5 mg weekly are available for further A1C reduction if needed after at least four weeks at 1.5 mg.

Safety profile & adverse events

Most common adverse events: nausea, diarrhea, vomiting, abdominal pain, decreased appetite, dyspepsia, fatigue. Boxed warning for thyroid C-cell tumor risk. Contraindications: MTC, MEN 2. Monitor: pancreatitis, hypoglycemia (with insulin or sulfonylureas), kidney injury, gallbladder events, hypersensitivity.

Cost & access in 2026

Trulicity list price is approximately $1,000 per month. Generally covered by commercial insurance and Medicare Part D for type 2 diabetes with prior authorization. Patient-assistance and savings-card programs available through Lilly.

Frequently asked questions

Dulaglutide vs semaglutide?

Both are weekly GLP-1s for type 2 diabetes. Semaglutide generally produces greater A1C reductions and weight loss at maximum doses. Dulaglutide's pen mechanism (no needle handling) is preferred by some patients.

Is dulaglutide effective for weight loss?

Weight loss is a secondary effect, typically smaller than that achieved with semaglutide or tirzepatide. Dulaglutide is not FDA-approved for weight management.

What makes the cardiovascular indication different?

REWIND included a broader population — patients with type 2 diabetes and CV risk factors but not necessarily established CVD. This contrasts with most other GLP-1 CV trials and broadens the eligible patient population.
